Pathway Plus is a pioneering project, funded by Brighton & Hove CCG as part of the integrated care programme. The project aims to improve the access and engagement with healthcare of those who are homeless or vulnerably housed and will work closely with specialist homeless healthcare provider Arch Health CIC.

We have an exciting opportunity to appoint 2 new Engagement Worker's who will provide support to people experiencing housing vulnerability to access and engage with health care providers. The project works with people upon discharge from hospital, or as referred by GP surgeries or the voluntary sector, to ensure that their health recovery is sustained and that they receive relevant assistance with their broader needs, linking in with the wide range of services which are available in the city. Specifically, the work will involve liaising with stakeholders and taking on a caseload of homeless patients at the point of discharge from hospital or referral. Once referrals are received, appropriate care plans will be established and this team will provide non-clinical floating support to individuals.

Regular activities will be connecting clients with appropriate support services, accompanying them to appointments, assisting them in addressing wider health needs such as drug and alcohol use. Staff will also be involved in the wider work of Justlife in Brighton and Hove providing support to people who live in emergency accommodation and vulnerably housed situations.  We are looking for an Engagement Worker who is highly motivated and has some experience of working in a related field in a voluntary or paid capacity.

Salary:                                           £18,226 to £19,728 PA 

This role requires an enhanced Disclosure and Barring Service (DBS) check and a barred list check.

Employment conditions are detailed in the employment contract, which you can view.  Annual leave entitlement is 25 days plus bank holidays (pro rata for part time staff and fixed term contract staff).

Justlife will provide relevant training opportunities for this role. Personal coaching will be available to you throughout your employment. Performance reviews will take place no less than twice each year.
