Through the Integrated Prevention and Earliest Help System, we will work with children, families, and young people to help ensure that every child, family and young person is supported to achieve his or her potential. The foundations for future mental health, emotional control, intelligence and parenting ability are all set in place in the early years and the home surroundings, family life and parenting are all important factors in determining positive outcomes for children in the future. The Team provide an assessment and support service to the young carers, aged 0-25 years, and their families, across West Sussex. This can include: home visits, information and signposting or the provision of short breaks through groups, commissioned services or activities. The team also provides consultation and training to other professionals regarding the needs of young carers, including raising the awareness across Children’s and Adults’ services. The Family Support Worker – Young Carers works in partnership with parents/carers, young people and other professionals to support and strengthen families in caring for their own children, undertaking direct work with children, young people, parents and carers to achieve change. Specifically this role will: • Undertake home visits and complete specialist Young Carers and/or CAF assessments with young carers and their families. • Provide information and enable young carers to access support to meet identified needs, negotiating with Adults’ services as appropriate. • Work as part of a multi-professional team around the child or family. • Provide the link between the young carer and young carers groups/group workers with the aim of enabling young carers to attend on-going groups and events on a regular basis. To attend young carers groups and events as and when necessary. • Identify and work creatively to minimise any barriers to young carers accessing a break from their caring role. • Undertake direct work with young carers and their families with the aim of minimising the impact of caring responsibilities on the young person. • Act as a Lead Professional, co-ordinating and monitoring packages of support provided by the Team around the Child/Team and Family. • Assist other members of the Young Carers team with raising awareness of young carers’ issues and needs.
